Summer 2018 Status Change Notification
Submit this form to inform the Financial Aid & Scholarship Department of changes to your summer enrollment. Your summer financial aid eligibility will be reviewed based on your current enrollment. This form can also be used to decline your summer financial aid award.

2017-2018 Confirmation of Update to Citizenship
Submit this form to confirm that you have updated your U.S. citizenship status with the Social Security Administration. You must also present, in person, to the Financial Aid & Scholarship Department your original citizenship document as proof of your U.S. citizenship status. 2017-2018 Selective Service Registration
Submit this form to notify the Financial Aid & Scholarship Department of your Selective Service registration status. Male students between the ages of 18 and 25 are required to register with Selective Service before financial aid eligibility can be determined. You must attach proof of your Selective Service registration or proof of your exemption status. Log in to the myNorthridge Portal, go to the SOLAR Student Center and review your To-Do List to see if the form is requested. 2017-2018 Statement of Educational Purpose
Submit this form so that the Financial Aid & Scholarship Department can verify your identity and collect a statement of educational purpose as required by the U.S. Department of Education. You must appear in person at the Financial Aid & Scholarship Department, present a valid government-issued photo ID and sign a statement of educational purpose attesting that the federal financial assistance you receive will be used for educational purposes only. If you are unable to appear in person, you must sign the statement, have a notary public sign it to confirm that you appeared and presented the ID confirming your identity, and submit it with a copy of your ID.
